Past due taxes and survivors social security benefits?

Your mother’s benefits will be permanantly reduced because the income tax return is the source document for crediting a self-employed person’s social security account. In her case, the last two years will be figured in at zero.

Were the W-2’s done properly for your father’s work?

No, her benefits (assuming she qualifes) will not be denied due to past taxes. However, a percentage of the payment may offset to pay any taxes she may be liable for.
